Goiter – What are the signs and symptoms, relevant dDx., sequelae, work up, and treatment options?

Goiter is diffuse thyroid enlargement without nodularity caused by iodine deficiency, fluoride, and diet (brassica, soy, licorice)

Sn. & Sx.:

  • Dry cough
  • Dyspnea
  • Dysphagia
  • Stridor
  • Thyroid sensitive to touch
  • Palpable thyroid without nodularity

dDx.:

  • Thyroiditis
  • Toxic adenoma, multinodular goiter
  • Thyroid cancer

Sequelae:

  • Airway obstruction
  • Multinodular goiter
  • Thyrotoxicosis

Work up:

  • Thyroid function tests
  • Imaging (ultrasound)

Tx.:

  • Radioactive iodine therapy
  • Surgical resection
